Home Health Registered Nurse | Benton, Louisiana in Benton, LA
Job Description
An experienced Home Health Registered Nurse (RN) is needed to provide skilled nursing care in patients homes, ensuring personalized, high-quality healthcare. This full-time, 13-week contract role involves visiting patients within a designated territory, where clinical expertise and independent decision-making are essential for delivering patient-centered care and promoting positive health outcomes.
Key Responsibilities:
• Perform comprehensive OASIS assessments and develop individualized care plans.
• Administer skilled nursing interventions such as wound care, medication management, IV therapy, and chronic disease monitoring.
• Monitor patient progress, detect changes in condition, and communicate effectively with the interdisciplinary care team and physicians.
• Educate patients and families on treatment plans, medication use, disease management, and preventive health.
• Document care accurately and promptly using Home Care Home Base (HCHB) electronic records.
• Collaborate with therapists, social workers, and home health aides to ensure cohesive care delivery.
• Participate in case conferences, staff meetings, and ongoing professional development.
• Uphold all regulatory and agency standards for both documentation and patient care.
Qualifications:
• Active RN license in good standing.
• Minimum one year recent home health experience, preferably field-based.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Strong clinical judgment, critical thinking, and organizational skills.
• Reliable transportation with valid drivers license and auto insurance.
• Ability to work independently within a flexible, field-based schedule.
• BSN preferred.
• Familiarity with Medicare and home health compliance preferred.
• Experience managing 2532 points weekly preferred.
Schedule:
• Monday through Friday, 8:00 AM 5:00 PM
• Caseload: Typically 35 patient visits per day (2532 points weekly)
