Key Responsibilities:
• Provide skilled nursing care and interventions in patients homes
• Conduct thorough patient assessments and complete OASIS documentation accurately and punctually
• Collaborate with physicians and interdisciplinary care teams to develop and implement individualized care plans
• Perform wound care, medication management, patient education, and disease monitoring
• Communicate patient status updates and changes in condition to the clinical team promptly

Qualifications:
• Current and active RN license
• Minimum one year of recent experience as a home health RN
• Required experience with OASIS assessments
• Proficiency with HomeCare HomeBase is strongly preferred
• Ability to manage a structured full-time schedule with daily patient visits
• Reliable transportation and a valid drivers license

Schedule:
• Full-time, Monday through Friday
• On-call availability likely required
