Department: Home Health RN Shift: Day Working Hours: 8:00 AM - 4:30 PM Summary: Join our dedicated team at Baptist Health as a Home Health/Hospice RN Case Manager, where you will provide, manage, and coordinate comprehensive care for an assigned caseload of patients in the comfort of their homes.
