As a CenterWell Home Health nurse, you'll play a crucial role in transforming patients' lives through personalized, one-on-one care. Your efforts will assist patients in regaining their independence, all while providing comfort from their own homes. Collaborate with a dedicated team of physicians and clinicians to create and manage comprehensive care plans designed to support recovery and reintegrate patients into a fulfilling life.
Key Responsibilities:
• Conduct admissions, case management, and follow-up skilled nursing visits for patients receiving home health services.
• Deliver ongoing care and case management to ensure appropriate follow-up as directed by the Clinical Manager.
• Work closely with physicians to develop initial treatment plans based on their orders and your assessments.
• Provide hands-on care, manage and evaluate care plans, and educate patients and families to promote recovery.
• Reassess and update care plans in consultation with physicians, ensuring compliance with policies and regulations.
• Coordinate care involving various healthcare professionals, including Physical Therapists, Occupational Therapists, Home Health Aides, and other providers.
• Continuously report on patient care, progress, and conditions to the patient's physician and Clinical Manager.
• Implement tailored care plans with patients and families to achieve optimal health outcomes.
• Oversee and guide Home Health Aides, providing clear personal care instructions aligned with current care plans.
• Ensure accurate documentation by home health aides and personal care workers as part of your supervisory responsibilities.
• Consult with the physician and Clinical Manager to facilitate patient discharges, completing required clinical documentation.
• Create and maintain detailed medical documentation for all patients, including case conferences, patient contacts, medication changes, progress updates, and modifications to care plans.
Required Experience and Skills:
• Diploma, Associate, or Bachelor Degree in Nursing.
• A minimum of one year of nursing experience is preferred, with a background in med surg, ICU, ER, or acute care.
• Home health experience is an advantage.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Strong organizational and communication skills.
• A valid driver's license, auto insurance, and reliable transportation are required.
Compensation and Benefits:
We offer a competitive pay range of $45.00 - $63.00 per visit/unit, with an annual base pay between $70,500 - $96,900. Scheduled weekly hours are 40, resulting in an annual pay range of $77,200 - $106,200.
