As a Registered Nurse Case Manager (RNCM), your responsibilities will include:
• Conducting in-person patient interviews and comprehensive physical assessments.
• Managing and overseeing the implementation and continuous assessment of the patient's care plan, including coordination with home health aides, LPNs, RNs, and other caregivers.
• Communicating patient conditions and working collaboratively with relevant providers to adapt care as patient needs change.
• Educating patients and families on effective home health care practices, such as wound care, IV administration, and medication management.
• Focusing on reducing readmissions through promoting preventive care and ensuring continuity of care.
To qualify as a Registered Nurse Case Manager (RNCM) with us, you will need:
• A valid, unrestricted license to practice as a Registered Nurse (RN) in the associated state.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation for travel to care sites and work locations.
