Home Health Registered Nurse (RN) Case Manager
Location: Pueblo, CO
Status: Full Time
Visit Rates: $50-$130 per visit; $37 per point for the first 4 weeks during training

As a Home Health Registered Nurse (RN) Case Manager, you will:
• Complete initial and ongoing nursing assessments of in-home clients within specified deadlines.
• Perform assessments and procedures that require specialized knowledge, judgment, and skill based on principles of various sciences.
• Collaborate in developing and evaluating patient care plans in partnership with the patient, caregivers, and representatives.
• Oversee care plans for your case load through ongoing interdisciplinary care coordination.
• Engage in continuous assessment of patient needs.
• Conduct home visits to evaluate the quality of care provided and review clinical notes.
Must-Haves for Home Health Registered Nurse (RN) Case Manager:
• Graduate of an accredited nursing program with an active RN license in Colorado.
• A minimum of one (1) year of nursing experience, ideally in home healthcare.
• CPR Certification (demonstration course required).
• Ability to pass a background check and drug test (including THC testing).
• Familiarity with state and federal home health regulations.
• Clinical proficiency, strong communication skills, and compassion.
