Key Responsibilities for Home Health RN Case Managers:
• Deliver personalized home-based care to patients recovering from illness, injury, or chronic conditions
• Collaborate with physicians and supervise LPNs, CNAs, and aides providing care
• Conduct patient assessments, document progress, and communicate any changes to the physician
• Educate patients about their care plans, medications, therapies, dietary needs, and exercise
• Guide family members on patient safety and medication management
• Coordinate discharge plans to ensure goals are met

Requirements to Join Our Team:
• Active, unrestricted RN license in Michigan - BSN is not a requirement
