Collaborating with a dedicated team of physicians and clinicians, you will develop and manage care plans that promote recovery and help patients return to the lives they cherish. As a Home Health RN Case Manager, your responsibilities will include: • Conducting admission, case management, and follow-up skilled nursing visits for home health patients. • Providing continuous care and management for each patient, with necessary follow-up as directed by the Clinical Manager. • Collaborating with physicians to establish the initial treatment plan based on physician orders and initial patient assessments. You will deliver hands-on care, assess and adjust the care plan as per the physician's guidance, and ensure compliance with policies and regulations. • Coordinating care with a range of healthcare professionals including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers. • Regularly updating the patient’s physician and Clinical Manager on their care, condition, and progress. • Implementing care plans alongside patients and families to help optimize their health outcomes. • Overseeing and guiding Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing them with written instructions that reflect the current care plan. Monitoring the accurate completion of documentation by aides is also part of your leadership responsibilities. • Facilitating patient discharges in collaboration with the physician and Clinical Manager, including preparation and completion of required clinical documentation. • Generating necessary medical documentation on all patients, encompassing case conferences, patient contacts, medication order changes, re-certifications, progress notes, and any modifications to the care plan. This includes preparing visit reports, summarizing patient records, and consulting with other healthcare professionals to ensure the best patient care. Use your expertise to make a lasting impact! Qualifications: • Diploma, Associate, or Bachelor’s Degree in Nursing. • A minimum of one year of nursing experience is preferred. • Strong experience in med-surg, ICU, ER, or acute care. • Home health experience is a plus. • Current and unrestricted Registered Nurse licensure. • Current CPR certification. • Excellent organizational and communication skills. • A valid driver’s license, auto insurance, and reliable transportation are essential. Compensation: Pay Range: $45.00 - $63.00 per visit/unit with a yearly base salary of $70,500 - $96,900. Work Schedule: 40 hours per week.
