Provide skilled nursing care in a home health setting, delivering compassionate and professional services to patients in the Dekalb, IL area. This contract position supports patients with diverse medical needs, requiring at least one year of recent acute care nursing experience to ensure high-quality care and patient safety. Responsibilities: • Perform comprehensive nursing assessments and develop individualized care plans. • Administer treatments and medications as prescribed, including IV therapy when applicable. • Monitor patient progress and communicate effectively with interdisciplinary teams. • Provide education and support to patients and their families on managing health conditions. • Maintain accurate and timely documentation in compliance with regulations. • Collaborate on wound care and cardiac patient management if certified. • Travel to patient homes within the designated service area; mileage reimbursed. Qualifications: • Active Illinois RN license required. • Minimum of one year nursing experience in an acute care setting within the last three years. • Preferred experience in home health, IV therapy, emergency room, or oncology nursing. • Cardiac or wound care certification is a plus. • Current BLS certification required. • Ability to work an 8:00 AM to 5:00 PM schedule, Monday through Friday, with rotating holidays and every third weekend.
