An opportunity is available for a skilled Home Health Registered Nurse (RN) to deliver compassionate, patient-centered care within a community setting. This contract role entails providing comprehensive nursing services to patients in their homes, focusing on clinical excellence and effective communication to support positive health outcomes. Key Responsibilities: • Conduct thorough assessments using OASIS and develop individualized care plans aligned with physician directives. • Administer skilled nursing care, including wound management, medication and IV therapy, and chronic disease oversight. • Monitor patient progress, identify changes in condition, and communicate effectively with healthcare teams. • Educate patients and families about their conditions, treatments, medications, and preventive health strategies. • Maintain accurate documentation with Home Care Home Base (HCHB) software. • Collaborate with multidisciplinary team membersphysical therapists, occupational therapists, medical social workers, and home health aidesto coordinate care. • Participate in regular case conferences, staff meetings, and ongoing education. • Comply fully with all regulatory standards governing home health care and documentation. Qualifications: • Active and valid RN license in the state of practice. • Minimum of one year recent home health nursing experience, preferably in a field-based setting. • Proficiency with Home Care Home Base (HCHB) electronic medical records and OASIS assessment tools. • Strong clinical assessment capabilities and critical thinking skills. • Reliable transportation, valid drivers license, and insurance to support travel to patient homes. • Excellent communication, organizational, and time management skills. • BSN preferred. • Experience managing caseloads of 2532 points weekly and knowledge of Medicare home health compliance standards is advantageous. Schedule and Location: • Full-time, 13-week contract. • Monday through Friday, 8:00 AM to 5:00 PM. • Field-based role requiring daily travel within the assigned territory; mileage is reimbursed.
