As a Home Health Registered Nurse, you will provide direct nursing care to patients in their own homes, following physician orders and adhering to applicable laws, regulations, and agency policies. You will collaborate with healthcare professionals to ensure a seamless and comprehensive care plan, maintaining continuity of care by coordinating with physicians, agency personnel, patients, families, and community resources.

Key Responsibilities:
• Clinical Excellence
• Develop and revise individual care plans in collaboration with physicians and other healthcare professionals.
• Supervise and support home health aides and licensed practical/vocational nurses, assigning tasks as per state and federal regulations.
• Conduct required supervisory visits to ensure high-quality care delivery.
• Documentation and Care Delivery
• Provide high-quality clinical services within the designated scope of practice, ensuring compliance with infection control standards.
• Conduct comprehensive assessments (OASIS) and medication reconciliations promptly and accurately.
• Document patient visits according to policy and payer requirements in a timely manner.
• Quality Care
• Perform initial nursing evaluations to ensure patients meet eligibility and medical necessity, developing appropriate care plans in alignment with state guidelines.
• Effectively communicate vital patient information with the agency staff, including visit assignments and schedule changes.
• Engage with physicians, patients, and families to guarantee exceptional care and service standards.
• Team Collaboration
• Work under the guidance of the Clinical Director and Executive Director, completing tasks and training assignments promptly.
• Assist in onboarding new agency personnel and serve as a mentor to staff and students.
• Participate actively in quality improvement initiatives and on-call rotations to meet patient needs.

Required Qualifications:
• Current and unrestricted RN licensure in the state of practice.
• At least 1 year of Home Health experience.
• Valid Driver's License, vehicle insurance, and reliable transportation.
• Current CPR Certification.
• Ability to provide care in diverse home environments regardless of socio-economic status.

Preferred Qualifications:
• Ability to work independently.
• Strong communication, writing, and organizational skills.

Compensation is influenced by various factors such as local labor markets, education, and experience. Along with competitive hourly pay ranging from $28.27 to $50.48, we offer a comprehensive benefits package, including incentive programs, stock purchase options, and 401k contributions (subject to eligibility).
