Responsibilities include conducting comprehensive OASIS assessments, developing and managing individualized care plans, and providing skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management. You will collaborate closely with interdisciplinary team members, including physical therapists and social workers, to ensure coordinated care and positive patient outcomes. Accurate documentation in the Home Care Home Base electronic system and communication with physicians and care teams are essential components of the role. Key Qualifications and Desired Experience: • Active RN license in good standing with at least 1 year of recent home health experience, preferably field-based • Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools • Strong clinical assessment abilities, critical thinking, and independent time management skills • Reliable transportation, valid drivers license, and auto insurance • Excellent communication and organizational skills • BSN preferred • Knowledge of Medicare and home health compliance standards is a plus • Experience managing 2532 points per week caseload is advantageous Schedule/Hours: • Monday through Friday, 8:00 AM 5:00 PM • Typically 35 patient visits daily within assigned territory, with mileage reimbursement Benefits include weekly pay, options for medical, dental, and vision insurance, 401(k) retirement plans, 24/7 professional support, and the potential for contract extensions and renewals.
