Your Role as an RN Case Manager As an RN Case Manager, you will play a pivotal role in delivering quality home health care. You will work with a diverse patient population, ensuring they receive the best possible care tailored to their needs. Qualifications: • Graduate from an approved school of professional nursing and currently licensed as a registered nurse. • At least one (1) year of experience as a registered nurse. Required Certifications: • Valid state RN license. • Current CPR certification. • Valid driver’s license and insured vehicle in good working condition for travel to patient locations.
