Your role entails delivering skilled nursing services and coordinating home health care for assigned patients, ensuring quality and compliance with laws, regulations, and policies.
Key Responsibilities:
• Conduct nursing assessments and develop, implement, and evaluate care plans in collaboration with patients, caregivers, and physicians.
• Regularly assess the effectiveness of care plans, adjusting as needed based on patient responses and outcomes.
• Utilize the OASIS data set for ongoing assessments at appropriate intervals.
• Deliver specialized nursing services, such as medication administration, wound care, therapeutic treatments, and patient education.
• Engage patients and their families in health teaching and counseling, fostering independence in self-care techniques and disease prevention.
• Participate in health promotion and preventive activities.
• Initiate nursing procedures following the physician's care plan.
• Coordinate services and scheduling with the Clinical Team Manager and Clinical Team Coordinator, advising on any additional needs within 24 hours of Start of Care.
• Continuously assess and communicate patient conditions to the Clinical Team Manager, executing interventions as required.
• Handle care management duties, including notifying physicians of changes in conditions, obtaining orders, and planning for patient discharge.
• Document all communication accurately with the patient, family, and healthcare team members.
• Prepare care plans for Home Health Aides, when relevant.
• Communicate effectively in a positive and cooperative manner.
This position is field-based, requiring travel throughout San Antonio and surrounding counties to support our patients. On-call responsibilities (during weekdays and weekends) are expected, with mileage reimbursement and shift differentials provided.

Required Qualifications:
• Graduate from an accredited nursing school.
• Current unrestricted Texas RN license or compact license.
• IV Certification.
• BLS certification.
• Minimum of 1 year experience in home health care or a related setting.
• Experience with wound care.
• Proficient computer skills.
• Able to move and transfer patients safely.
• Access to reliable transportation for travel to client sites.
• Active driver’s license.
Preferred Qualification:
• Bilingual in Spanish.
Compensation for this role considers various factors, including local labor markets, education, experience, and certifications. The hourly rate ranges from $28.27 to $50.48 for full-time employees.
