Professional Case Management, a respected leader in home healthcare, is actively recruiting Registered Nurses to provide in-home healthcare services to veterans.

Available Hours:
• Friday - Sunday: 12-Hour Day Shifts
• Tuesday - Sunday: 12-Hour Night Shifts
Key Responsibilities:
• Proactively improve the quality of client care and independently conduct client visits to deliver skilled interventions.
• Provide direct care and delegate functions according to competence and education.
• Advocate for clients and assess their health status through thorough information gathering.
• Collaborate with clients, families, and healthcare teams to identify needs and establish realistic care goals.
• Regularly review and modify care plans based on client responses and needs.
• Plan interventions that support hygiene and ensure a safe, comfortable environment.
• Verify medical orders for accuracy and administer prescribed medications and IV therapy.
• Educate clients and families about care and self-administration techniques.
• Evaluate home environments for safety and implement plans for accident prevention.
• Maintain accurate client records and communicate significant changes in client conditions.
• Participate in team meetings and contribute to continuous professional self-development.

Qualifications:
• Graduate from a state-approved nursing school.
• Current, unrestricted RN license in the practice state.
