Job Description
An opportunity is available for a skilled Home Health Registered Nurse (RN) to provide comprehensive nursing care to patients within their homes. This role emphasizes delivering patient-centered, high-quality care while working independently in a field-based setting.
Responsibilities include conducting thorough OASIS assessments and developing tailored care plans to meet individual patient needs. The RN will provide skilled nursing services such as wound care, medication administration, IV therapy, and managing chronic conditions. Monitoring patient progress, recognizing changes in health status, and effectively communicating with healthcare teams and physicians are critical components of this position. Education and support for patients and their families on treatment plans and preventive care are also essential. Accurate documentation using Home Care Home Base (HCHB) is required, along with collaboration with therapists, social workers, and home health aides to ensure coordinated care delivery. Participation in team meetings and adherence to regulatory standards are part of the roles ongoing duties.
Qualifications include an active RN license in good standing with the state of Kansas, a minimum of one years recent home health experience (field experience preferred), and proficiency in the HCHB electronic documentation system alongside experience with OASIS assessments. Reliable transportation, a valid drivers license, and excellent communication and organizational skills are mandatory. A Bachelor of Science in Nursing (BSN) and knowledge of Medicare compliance are preferred.
This full-time contract position spans 13 weeks, scheduled Monday through Friday from 8:00 AM to 5:00 PM, typically managing 35 patient visits daily, equating to a caseload of 2532 points per week. Mileage reimbursement is provided for daily travel within the assigned territory.
