As a Home Health Registered Nurse, you will play a crucial role in providing and overseeing nursing care to patients in their homes, following physician orders and adhering to all legal and regulatory guidelines. You will coordinate the overall care plan with other healthcare professionals, ensuring seamless communication with patients, families, and community resources.

Key Responsibilities:
• Clinical Competence
• Develop and implement tailored care plans in collaboration with physicians and healthcare teams.
• Oversee the care provided by home health aides and LPNs, providing guidance and assigning tasks per regulations.
• Conduct mandatory supervisory visits as needed.
• Documentation and Care Delivery
• Deliver high-quality clinical services, adhering to infection control standards and the plan of care.
• Perform comprehensive assessments (OASIS) and ensure timely and accurate documentation.
• Document patient visits according to policy and payer requirements, syncing information promptly.
• Quality
• Conduct initial or comprehensive evaluations to confirm home health eligibility, establish care focus, and develop care plans.
• Effectively communicate relevant patient information with the care team and any schedule alterations.
• Ensure clear and timely communication with physicians, patients, and families for optimal care outcomes.
• Teamwork
• Follow direction from leadership and complete tasks efficiently, including required training.
• Support the onboarding process for new team members and mentor other staff and students.
• Engage in performance improvement initiatives and processes to enhance patient safety and outcomes.
• Participate in on-call and weekend rotations as needed.
• Contribute to the agency's utilization management efforts.

Required Qualifications:
• Current and unrestricted RN licensure.
• 1+ years of experience in Home Health.
• Valid Driver's License, vehicle insurance, and reliable transportation.
• Current CPR certification.
• Ability to work effectively in diverse home environments.

Preferred Qualifications:
• Ability to work independently.
• Excellent communication, documentation, and organizational skills.

The pay for this role ranges from $28.27 to $50.48 per hour, based on full-time employment and various factors including local market rates and your qualifications.
