Responsibilities
The Home Health Registered Nurse serves as the main contact for all care disciplines involved in patient treatment and is responsible for overseeing the schedule of visits throughout the care episode. Key responsibilities include:
• Coordinating and performing patient visits
• Consulting with physicians regarding patient care
• Ensuring accurate OASIS data collection
• Maintaining current medication profiles
• Timely reporting of lab results to physicians
• Attending weekly case conferences and monthly case manager meetings
Qualifications
To be considered for this role, Registered Nurses (RNs) must meet the following qualifications:
• Current RN license in the state of employment
• A minimum of one year of clinical experience is preferred
• Demonstrated knowledge and skill in current nursing practice
• Valid driver's license and automobile liability insurance
