As a Home Health Registered Nurse, you will play a crucial role in providing and coordinating high-quality nursing care in patients' homes, as per physician orders and in compliance with pertinent laws, regulations, and agency policies. You will collaborate with other healthcare professionals to create a comprehensive care plan, ensuring continuity and excellence in patient services.

Key Responsibilities:
• Clinical Competence: Develop, implement, and adjust plans of care with the physician and healthcare team. Supervise and instruct home health aides and LPNs/LVNs in accordance with regulations.
• Documentation and Care Delivery: Deliver clinical services that meet safety and infection control standards. Perform thorough assessments, including medication reconciliation, and document patient visits in line with policy.
• Quality Assurance: Conduct nursing evaluations to verify home health eligibility and necessity, while submitting timely, accurate documentation. Communicate effectively with agency staff, patients, and family members.
• Team Collaboration: Take guidance from Clinical and Executive Directors, assist with training new staff, and participate in performance improvement initiatives. You may also engage in on-call duties as required.

Required Qualifications:
• Active and unrestricted RN licensure in your state.
• Minimum of 1 year experience in Home Health.
• Valid Driver's License and access to a reliable vehicle or public transport.
• Current CPR certification.
• Ability to provide care regardless of patient's circumstances.

State-Specific Requirements: For Louisiana, a nurse must have 1+ years of clinical experience as an RN, which may be waived for those with recent LPN experience.

Preferred Qualifications:
• Ability to work independently and effectively.
• Strong communication, writing, and organizational skills.
