Key Responsibilities:
• Provide thorough nursing care in patients homes, including initial and ongoing assessments.

• Develop and execute individualized care plans tailored to patient needs.
• Educate patients and families about disease management, treatment options, and prevention.
• Collaborate closely with physicians, therapists, and other healthcare professionals.
• Accurately document all care activities in electronic medical records (EMR).
• Report significant changes in patient conditions promptly.
• Maintain adherence to all relevant state and federal home health regulations.

Qualifications:
• Active Registered Nurse (RN) license.
• Minimum of three months home health nursing experience, specifically with OASIS documentation.
• Experience with Home Care Home Base (HCHB) electronic charting is highly advantageous.
• Proficiency in clinical skills such as IV therapy, wound care, and medication management.
• Strong communication and interpersonal abilities.
• Self-motivated with excellent time management skills.
• Valid drivers license and dependable transportation.
