As a Home Health nurse with CenterWell, you will have the opportunity to make a significant impact in the lives of patients by providing individualized care in the comfort of their own homes. Collaborating with a team of dedicated clinicians and physicians, you will develop and implement care plans aimed at supporting patient recovery and promoting independence. Your Responsibilities as a Home Health RN Case Manager: • Conduct admissions, manage cases, and provide follow-up nursing visits to home health patients. • Administer ongoing care and case management, adhering to guidelines provided by the Clinical Manager. • Work closely with physicians to formulate initial treatment plans based on assessments and physician orders. • Deliver hands-on care, manage care plans, and educate patients, all while following the supervision of the Clinical Manager. • Regularly update care plans in consultation with physicians based on continuous evaluation and compliance with policies. • Coordinate patient care by collaborating with various healthcare professionals, including Physical Therapists, Occupational Therapists, and Home Health Aides. • Continuously report patient conditions and progress to the physician and Clinical Manager. • Involve patients and their families in the care plan to achieve optimal outcomes. • Supervise and guide the work of Home Health Aides and Personal Care Workers, providing clear care instructions that reflect the current care plan. • Ensure proper documentation by aides and workers as part of your supervisory duties. • Facilitate patient discharge in consultation with the physician and Clinical Manager, ensuring all documentation is complete. • Prepare all necessary medical documentation for patient interactions, medication changes, progress updates, and care plan modifications. Required Experience and Skills: • Diploma, Associate, or Bachelor's Degree in Nursing. • A minimum of one year of nursing experience in Home Health. • Proficiency in OASIS and experience with Home Care Home Base. • Current and unrestricted Registered Nurse license. • CPR certification in good standing. • Strong organizational and communication abilities. • Valid driver's license, auto insurance, and reliable transportation are essential. Pay and Benefits: Pay ranges between $45.00 and $63.00 per visit/unit, with an annual base salary between $70,500 and $96,900. Scheduled weekly hours are 40.
