Responsibilities of a Home Health Registered Nurse (RN):
• Provide home-based care to patients recovering from illnesses, injuries, and managing chronic diseases, often following discharge from a hospital, rehab, or nursing facility.
• Collaborate with physicians and supervise a multidisciplinary team in delivering patient care.
• Conduct patient assessments, document progress, and communicate any changes to their physician.
• Educate patients about their care plans, medications, therapy, diet, and exercise.
• Coach family members on the patient's care plan, medications, and home safety.
• Ensure healthcare goals are met and coordinate patient discharge from services.

Essential Qualifications for Home Health Registered Nurses (RN):
• Graduate from an accredited Registered Nurse (RN) program
• Active, unrestricted Registered Nurse (RN) license in South Carolina or a compact multi-state license
• A minimum of one year of nursing experience, preferably in home healthcare, Med/Surg, or Critical Care
