Job Description
A rewarding opportunity is available for an experienced Home Health Registered Nurse (RN) to deliver exceptional skilled nursing care in patient homes. Key Responsibilities:
• Conduct thorough OASIS assessments and develop tailored care plans.
• Provide skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes in condition, and communicate promptly with physicians and the interdisciplinary care team.
• Educate patients and families on disease management, treatments, and preventive care.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate effectively with therapists, social workers, and home health aides to ensure comprehensive care coordination.
• Participate in case conferences, staff meetings, and ongoing education to stay current with best practices.
• Adhere strictly to regulatory and agency standards in both care delivery and documentation.
Qualifications:
• Active RN License in good standing within the state.
• Minimum one year of recent home health nursing experience, preferably in the field.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Strong clinical assessment and critical thinking abilities.
• Reliable transportation with a valid drivers license and auto insurance.
• Self-motivated with excellent communication, time management, and organizational skills.
Preferred:
• BSN degree.
• Prior experience managing 2532 points weekly.
• Understanding of Medicare and home health compliance standards.
Schedule & Location:
• Full-time contract for 13 weeks.
• Monday through Friday, 8:00 AM 5:00 PM.
• Daily travel to patient homes within the local territory with mileage reimbursement.
