Your Responsibilities:
• Deliver attentive home-based care to patients recovering from illnesses, injuries, or managing chronic diseases.
• Collaborate with a team of physicians and manage a multidisciplinary approach to patient care.
• Conduct thorough patient assessments, document care progress, and communicate changes to physicians.
• Educate patients about their care plans, medications, therapies, and lifestyle modifications.
• Support and coach family members regarding care protocols and home safety.
• Coordinate effective discharge from services while ensuring all patient goals are achieved.

Required Qualifications:
• Graduate from an accredited Registered Nurse (RN) program.
• Possess an active, unrestricted Registered Nurse (RN) license in South Carolina or a compact multi-state license.
• At least one year of nursing experience, preferably in home healthcare, Med/Surg, or Critical Care.
