Job Responsibilities This is NOT your typical corporate RN position. We are seeking a motivated, compassionate Home Health Registered Nurse (RN) who values autonomy, flexibility, and patient-centered care. In this role, you’ll work independently in the community, caring for adults and seniors with a wide variety of diagnoses and functional levels. This position is Part-Time or PRN, with a requirement of approximately 10 visits per week within your assigned service area. • Deliver comprehensive OASIS Assessment to patients in their homes, ensuring adherence to individualized care plans. • Monitor patient health status and vital signs, documenting changes and reporting concerns to the healthcare team. • Collaborate with interdisciplinary teams to coordinate care and support for patients with diverse needs. • Provide education and support to patients and their families regarding health management and treatment plans. • Utilize electronic health record systems such as KINNSER for accurate documentation and communication. • Maintain a safe environment for patients by adhering to infection control protocols and safety guidelines. • Job Requirements • Degree from accredited school or program • Current State licensure as a nurse • CPR or BLS certification • Car insurance & Registration • Fingerprint Clearance Card • Must be physically and mentally able to perform the essential job functions: lifting and moving patients, pushing and pulling equipment in potentially awkward or strained positions Job Perks • Bonus pay for 5 star reviews • Flexibility - Build your own schedule!
