General Summary of Duties: As a Home Health Registered Nurse, you will provide essential nursing care to patients in their homes, following a collaborative plan established with their physician. You’ll play a key role in improving patient outcomes and ensuring adherence to agency policies and procedures. Supervision Received: You will report directly to a Clinical Manager. Supervision Exercised: You will oversee Home Care Aides. Qualifications: • Graduation from an NLN-approved school of nursing with either an associate or baccalaureate degree. • At least one year of recent medical/surgical nursing experience is required; experience in community health nursing is preferred. • Current RN license to practice in the State of Connecticut. Essential Skills and Requirements: • In-depth knowledge of nursing theory and practice. • Familiarity with medical equipment used in patient care. • Ability to adapt and apply guidelines effectively. • Strong documentation skills and ability to maintain effective communication with colleagues and patients. • Access to reliable transportation and proof of appropriate automobile insurance is necessary. Key Responsibilities: • Provide comprehensive nursing services including assessments, care plan development, and patient education. • Ensure patient safety and ongoing evaluation of care. • Document services and manage care in compliance with regulations. • Effectively manage work assignments to meet productivity standards. • Collaborate with Home Health Aides and provide necessary supervision and evaluations. • Engage in continuous professional development and compliance with agency policies.
