
Home Health Registered Nurse in Clarkedale, AR
Job Description
As a Home Health Registered Nurse, you will play a crucial role in providing and directing nursing care to patients in their homes, as established by physicians and in compliance with applicable laws and agency policies. You will collaborate with healthcare professionals to coordinate patient care while maintaining continuity and ensuring that essential information is shared among all involved parties.
Primary Responsibilities:
• Clinical Competence
• Develop and revise patient care plans in partnership with physicians and healthcare teams.
• Oversee care provided by home health aides and licensed practical/vocational nurses, offering guidance and task assignments in accordance with state and federal regulations.
• Conduct necessary supervisory visits.
• Documentation and Care Delivery
• Deliver high-quality clinical services while adhering to infection control standards, ensuring compliance with the care plan and coordination with the healthcare team.
• Accurately complete comprehensive assessments, including medication reconciliation, in a timely manner.
• Document patient visits according to policy, ensuring timely synchronization per LHC policy.
• Quality
• Conduct initial and comprehensive nursing evaluations, ensuring all patients meet eligibility and medical necessity guidelines.
• Effectively communicate key information regarding patient care, visit assignments, and coordination among clinicians and staff.
• Ensure timely and effective communication with physicians, patients, and families to maintain high-quality care and service excellence.
• Teamwork
• Professionally take direction from the Clinical Director and Executive Director, completing assigned tasks promptly.
• Assist with the orientation of new personnel and mentor other staff and students.
• Engage in performance improvement initiatives and strive to reduce patient hospitalizations through best practice implementations.
• Participate in scheduled on-call and weekend rotations as needed.
• Adhere to the agency's utilization management model.

Required Qualifications:
• Current and unrestricted RN licensure.
• 1+ years of Home Health experience.
• Valid Driver's License, vehicle insurance, and reliable transportation.
• Current CPR Certification.
• Ability to provide care in diverse home situations regardless of age, race, creed, disability, or financial circumstance.
Preferred Qualifications:
• Ability to work independently.
• Strong communication, writing, and organizational skills.
Compensation depends on various factors, including local labor markets and individual qualifications. Alongside a competitive salary, we provide a comprehensive benefits package, including incentive programs, equity stock purchase options, and a 401k contribution, all subject to eligibility requirements. The hourly pay range for this role is $28.27 to $50.48 based on full-time employment.
