
Home Health Registered Nurse in Buncombe, IL
Job Description
As a Home Health Registered Nurse, you will play a crucial role in providing and directing nursing care to patients in their homes, following physician prescriptions and adhering to all relevant laws and agency policies. Your coordination skills will help ensure continuity of care by collaborating with physicians, agency personnel, patients, families, and community resources.
Key Responsibilities:
• Clinical Competence
• Initiate, develop, implement, and revise patient care plans in collaboration with physicians and healthcare professionals.
• Supervise care from home health aides and licensed practical/vocational nurses, providing instruction and assigning tasks as per State and federal regulations.
• Conduct required supervisory visits.
• Documentation and Care Delivery
• Deliver high-quality clinical services within the scope of practice, following infection control standards and coordinating with healthcare teams.
• Perform comprehensive assessments, including OASIS, ensuring accurate and timely medication reconciliation.
• Document patient visits in accordance with policy and payer requirements, ensuring timely syncing as per LHC policy.
• Quality Assurance
• Evaluate nursing needs during initial visits, confirming home health eligibility, and developing care plans with physicians.
• Communicate patient care issues, changes in schedule, and coordination with other clinicians effectively and timely.
• Ensure quality care by maintaining clear communication with physicians, patients, and their families.
• Team Collaboration
• Follow directions from the Clinical Director and Executive Director professionally, completing tasks punctually.
• Assist in orienting new staff and serve as a mentor to new team members.
• Engage in performance improvement initiatives, aiming to enhance patient safety and reduce unnecessary hospitalizations.
• Participate in on-call and weekend rotations as needed.
• Adhere to the agency's utilization management model.

Required Qualifications:
• Current and unrestricted RN licensure in your respective state.
• Minimum of 1 year of Home Health experience.
• Valid Driver's License and access to a dependable vehicle or public transportation.
• Current CPR Certification.
• Ability to deliver care to clients without regard to age, race, creed, color, sex, disability, or financial status.
Preferred Qualifications:
• Ability to work independently.
• Strong communication, writing, and organizational skills.
