Job Description
Registered Nurse – Home Health (Greater Indianapolis, IN Area)
Shift: Monday–Friday, 8:00 AM – 5:00 PM (No weekends required)
Coverage Area: Greater Indianapolis, Indiana
License: Active Indiana RN or Compact License required
Experience: Home Health and OASIS experience preferred
Mileage Reimbursement: $0.50 per mile
On-Call: Every 3 weeks — $25/day and $50 per weekend day or holiday
Job Summary
The Registered Nurse (RN) is responsible for providing high-quality home health care to individuals throughout the Greater Indianapolis area. This role includes assessing patient health, developing and implementing care plans, and promoting health maintenance and disease prevention. The RN will also provide case management and may supervise other nursing or support staff.
Responsibilities
• Observe, assess, plan, implement, and evaluate nursing support for individuals with acute, chronic, or long-term conditions in a home or community-based setting.
• Collaborate with physicians and interdisciplinary healthcare teams to design and execute personalized care plans.
• Coordinate communications between healthcare professionals and ensure continuity of care and safety for all patients.
• Manage medication administration, storage, and disposal according to established policies and physician orders.
• Ensure progress reports are made to physicians when a patient’s condition changes.
• Complete required documentation (daily, monthly, quarterly, annual) accurately and on time.
• Maintain thorough nursing notes to track patient status and outcomes.
• Ensure equipment, furnishings, and supplies meet health and safety standards to prevent disease transmission.
• Supervise and support Licensed Practical/Vocational Nurses as needed.
• Schedule and follow up on all health-related appointments to ensure timely care.
• Communicate with inpatient teams and discharge coordinators during hospitalizations to ensure smooth transitions of care.
• Provide health and safety training for new employees and annual recertification as required.
• Train unlicensed staff on patient-specific health, safety, and disease prevention needs.
Qualifications
• Active Indiana RN License or Compact License
• Minimum of 1–2 years of home health experience preferred
• OASIS documentation experience strongly preferred
• Strong organizational, communication, and leadership skills
• Ability to work independently and manage multiple cases effectively
