Reports To: Associate Director, Home Health Branch Administrator Work Schedule: Full-time/40 Hours Position Type: On-site Branch Location: 123 Example St, Kannapolis, NC • This is an on-site position located at our Kannapolis, NC branch. Remote, work-from-home, and hybrid work arrangements are not supported for this role. As the Home Health RN Clinical Supervisor, you will play a pivotal role in coordinating and overseeing all direct patient care services provided by our dedicated clinical team. Your primary focus will be on ensuring clinical quality assurance and optimizing home health operations to enhance patient well-being. Your exciting responsibilities include: • Developing, planning, and implementing clinical operations for the assigned branch. • Conducting thorough assessments and reassessments of patients while updating care plans in alignment with company and regulatory policies. • Managing caregiver assignments and ensuring delivery of high-quality patient care. • Reviewing case referrals and determining admission criteria to guarantee that patients receive the appropriate services. • Guiding clinicians in establishing therapeutic goals and developing effective patient Plans of Care (POC). • Monitoring documentation for compliance with regulatory agencies and managing audits for timely billing in accordance with Medicare regulations. • Facilitating effective communication among team members, attending physicians, and caregivers to optimize care. • Collaborating with the Branch Director and Finance Department to achieve budget and revenue goals. • Supervising clinical staff, ensuring adherence to policies, and addressing corrective actions as necessary. • Participating in the recruitment, hiring, training, and professional development of direct care clinicians. • Analyzing performance and implementing improvement strategies based on Performance Improvement principles. • Managing Quality Assurance/Performance Improvement activities in conjunction with Utilization Review staff. • Providing direct patient care in emergencies when required and stepping in as the Branch Director as needed. Education: Associate degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) strongly preferred. License/Certifications: Active and unrestricted Registered Nurse (RN) license or Compact State RN license valid in North Carolina. Current CPR and/or BLS certification required. Experience/Skills Required: • Minimum 2 years of experience as a Registered Nurse (RN). • Background in home-based care settings preferred, including home health, hospice, or palliative care. • Management experience is strongly preferred. • OASIS experience is highly desirable; OASIS certification preferred. • Experience with Homecare Homebase (HCHB) and knowledge of CMS PDGM is a plus. Additional Information: • Normal Hours of Operation: M-F / 8a-5p (ET) • On-Call Expectation: Yes, with a rotating shift. • Census Size: 506+ (3.0 STAR rating) • Annual Bonus Eligible: Yes, based on individual and company performance. • Pay Range: $77,200 - $106,200 per year This position is also eligible for a bonus incentive plan based on performance.
