Role Responsibilities: • Deliver nursing care in patients’ homes, focusing on home health and hospice needs. • Manage patient caseloads of approximately 5-6 per day. • Perform clinical documentation using HomeCare HomeBase (HCHB) system. • Collaborate with interdisciplinary teams to ensure comprehensive patient care. • Maintain adherence to dress code standards: nursing uniforms in navy or navy/white. • Utilize your own reliable vehicle to travel between patient homes safely. • Uphold patient safety and quality care standards in a home setting. Desired Experience and Qualifications: • Current Registered Nurse licensure with home health and hospice experience required. • Familiarity with Franklin and Macon County healthcare needs preferred. • Strong communication and critical thinking skills; ability to work independently. • Experience with the HCHB documentation system a plus. • Valid driver’s license and reliable vehicle necessary for patient visits.
