• Provide nursing care and education tailored to each patients needs.
• Manage case loads, coordinate with healthcare teams, and ensure compliance with agency standards.
• Document patient progress accurately and efficiently.
Qualifications and Desired Experience:
• Valid RN licensure.
• Proven experience in Home Health nursing and case management.
• Proficiency with OASIS assessments.
• Ability to adapt quickly and work independently.
• Preference for candidates with travel nursing experience.
• Availability to commit to the full duration of the contract with no requested time off.
