• Perform wound care, medication management, patient education, and monitor disease progression.
• Communicate any significant changes in patient conditions promptly to the clinical care team.
Qualifications and Desired Experience:
• Active Registered Nurse (RN) license.
• Minimum of one year of recent home health nursing experience.
• Proven experience with OASIS assessments.
• Proficiency in HomeCare HomeBase software is strongly preferred.
• Ability to manage a full-time schedule with daily patient visits.
• Reliable transportation and a valid drivers license are required.
Schedule:
• Full-time, Monday through Friday.
• On-call duties probable.
