In the role of Primary Care Nurse (RN), you'll apply your extensive training to deliver personalized care to patients in a fulfilling environment that promotes autonomy. You will be covering the following towns: Union, Ashford, Eastford, Chaplin, Hampton, and nearby areas. Responsibilities Include: • Conducting comprehensive assessments to create individualized care plans. • Facilitating effective communication between patients and the interdisciplinary team. • Developing and sustaining therapeutic relationships by offering education and encouraging self-advocacy and independence. • Utilizing state-of-the-art technology to document skilled services provided. Qualifications: • Registered Nurse with a valid CT RN license. • At least one year of RN experience; homecare experience is preferred. • Must possess a valid driver's license and have reliable transportation. • Computer proficiency is required.
