Key Responsibilities • Conduct thorough hospice admissions and comprehensive assessments for patients and their families. • Ensure precise documentation and create tailored care plans based on individual patient needs. • Deliver direct patient care, including medication administration and medical treatments as necessary. • Work collaboratively with physicians and interdisciplinary teams to devise personalized care strategies. • Act as an essential link between patients, families, and healthcare providers. • Educate patients and caregivers about disease progression, symptom management, and available hospice services.
