Are you a dedicated RN who thrives on making meaningful connections at critical moments? As a Hospice Admissions Nurse, you play a vital role in guiding families through difficult decisions, helping them embrace compassionate end-of-life support with clarity and kindness.
Key Responsibilities:
• Serve as an ambassador for our hospice services, reporting directly to the Admissions Manager.
• Conduct visits to patients and families in various settings, including homes, hospitals, or facilities, to assess eligibility for hospice care.
• Communicate the philosophy and services of hospice with clarity, compassion, and clinical expertise.
• Support patients and families throughout the admissions process, ensuring a seamless transition into care.
• Collaborate with healthcare professionals, including referring physicians and case managers, to facilitate smooth transitions of care.
• Utilize the nursing process to evaluate patient needs, assist in care planning, and accurately document clinical findings.
• Build trust and provide emotional support to patients and families during this significant decision-making process.
• Complete necessary documentation to ensure compliance with state and federal regulations.
• Work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care transitions.
Who You Are:
Qualifications:
• Active RN license in the state of employment (or eligible to obtain).
• At least 1 year of general nursing experience.
• A compassionate heart with a commitment to patient-centered care.
• Excellent assessment, communication, and interpersonal skills.
• Comfortable visiting patients in various settings, including hospitals and home environments.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
Preferred Experience:
• 2+ years in hospice, home health, healthcare admissions, or medical sales.
• Bachelor's Degree and/or Certified Hospice and Palliative Nurse (CHPN) certification.
• Background in case management or care coordination.
• Experience with electronic medical records and compliance documentation.
