As a Hospice Admissions Nurse, you will play a vital role in helping families navigate the path toward compassionate end-of-life support during challenging times. Your Responsibilities as a Hospice Admissions Nurse: • Report to the Admissions Manager, serving as a key ambassador for our hospice services. • Visit patients and families in various settings—homes, hospitals, or facilities—to assess eligibility for hospice care. • Clearly and compassionately explain hospice philosophy and services with clinical confidence. • Guide patients and their loved ones through the admissions process, ensuring a supportive transition to care. • Collaborate with referring physicians, case managers, and facility staff to ensure seamless transitions of care. • Utilize the nursing process to assess patient needs, develop care plans, and document clinical findings. • Build trust and provide emotional support to patients and families during this critical decision-making time. • Accurately complete documentation, ensuring compliance with state and federal regulations. • Work closely with the clinical and interdisciplinary team to guarantee timely admissions and quality handoff of care. About You: Qualifications: • Active RN license in the state of employment (or eligibility to obtain). • 1+ year of general nursing experience. • A caring approach: compassionate, empathetic, and dedicated to patient-centered care. • Strong assessment, communication, and interpersonal skills. • Comfortable visiting patients in hospitals, skilled nursing facilities, or home settings. • Valid driver's license, auto insurance, and reliable transportation. • Current CPR certification. Preferred Experience (Not Required): • 2+ years of experience in hospice, home health, healthcare admissions, or medical sales. • Bachelor's Degree and/or Certified Hospice and Palliative Nurse (CHPN) certification. • Background in case management, care coordination, or healthcare outreach. • Experience with electronic medical records and compliance documentation.
