You will support both Palliative Care and Hospice referrals. Key Responsibilities Include: • Providing comprehensive palliative care and hospice services through in-depth patient assessments • Consulting with patients and families on goals of care and communicating these to the care team • Conducting hospice consult reviews to determine patient appropriateness for hospice care • Assuming the role of hospice RN with hospital patients, developing care plans, and conducting follow-ups as needed • Acting as a clinical resource for the hospice and palliative team while mentoring new members • Consistently documenting patient care interventions in a thorough and timely manner Sign-on and Relocation Benefits are available for qualified candidates!
