What You'll Do as a Hospice RN / Case Manager:
• Serve as the primary contact for patients and families, guiding them through every phase of their hospice journey.
• Deliver and record skilled nursing care based on each patient's personalized Plan of Care, whether in their home, assisted living, or another familiar setting.
• Conduct initial and continuous assessments to monitor patient conditions and make necessary adjustments to care.
• Work collaboratively with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, and chaplains—to create and refine individualized care plans.
• Provide emotional, physical, and spiritual support to both the patient and their loved ones, offering education, comfort, and bereavement guidance.
• Educate caregivers on how to safely and confidently care for their loved ones, providing both practical advice and emotional reassurance.
• Communicate significant updates in patient conditions to the care team to ensure timely and appropriate interventions.
• Maintain accurate and current clinical documentation while adhering to infection control and compliance standards.
• Support organizational goals related to quality improvement and scheduling initiatives to ensure that care is consistent, compassionate, and efficient.
• Assist patients and families in navigating important topics such as:
• Medication administration
• Hospice philosophy and services
• Symptom and pain management
• End-of-life processes and expectations
About You:
• Active RN license in the state of employment (or eligible to obtain).
• At least 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.
• A compassionate and patient-centered approach to care.
• Comfortable providing care in diverse environments, including private homes and healthcare facilities.
• Excellent communication and critical thinking skills, especially in emotionally sensitive situations.
• Valid driver's license, auto insurance, and reliable transportation.
• CPR certification is required.
Preferred Experience (Not Required):
• Experience as a hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac)
• Exposure to admissions nursing, wound care, float pool, or travel nursing
• Experience working with terminally ill patients or in serious illness support
