Key Responsibilities as a Hospice RN / RN Case Manager:
• Serve as the primary contact for patients and families, guiding them through their hospice experience.
• Deliver skilled, hands-on nursing care, tailored to each patient's Plan of Care, in their home, assisted living, or other preferred settings.
• Carry out initial and ongoing assessments to monitor and adjust care based on patient needs.
• Collaborate with a multidisciplinary care team, including physicians, LPNs, CNAs, social workers, chaplains, and others, to create and refine personalized care plans.
• Provide emotional, physical, and spiritual support to both patients and their loved ones, offering education, comfort, and guidance during bereavement.
• Teach caregivers safe and confident care techniques, providing practical training and emotional support.
• Communicate significant updates regarding patient conditions to the care team, ensuring appropriate and timely interventions.
• Maintain accurate and current clinical documentation, adhering to infection control and compliance standards.
• Support quality improvement and scheduling initiatives to ensure that care is consistent, compassionate, and efficient.
• Assist patients and families in understanding and navigating critical topics such as:
• Medication administration
• Hospice philosophy and services
• Symptom and pain management
• End-of-life processes and expectations
What You Bring to the Role:
• Active RN license in the state of employment (or eligible to obtain).
• 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.
• A genuine commitment to hospice care: compassionate, empathetic, and patient-centered.
• Ability to provide care in diverse environments, including private homes and facilities.
• Strong communication and critical thinking abilities in emotionally sensitive situations.
• Valid driver's license, auto insurance, and reliable transportation.
• CPR certification required.
Preferred Qualifications (Not Required):
• Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac)
• Experience in Admissions RN, wound care, float pool, or travel nursing
• Exposure to terminally ill patients or support for serious illnesses
