What You'll Do as a Hospice RN / RN Case Manager: • Serve as the primary point of contact for patients and families, guiding them through every step of their hospice journey. • Deliver and document tailored nursing care according to each patient's individualized Plan of Care within their home, assisted living, or other familiar settings. • Perform thorough assessments to monitor patient conditions and make necessary adjustments to care plans. • Collaborate with an interdisciplinary team, including physicians, LPNs, CNAs, social workers, and chaplains, to build and refine highly personalized care plans. • Provide emotional, physical, and spiritual support not only to the patient but also to their loved ones, offering education, comfort, and guidance through bereavement. • Educate caregivers on safe and confident care practices for their loved ones, providing both practical training and emotional support. • Communicate vital updates regarding patient conditions to the care team to ensure timely interventions. • Maintain accurate and up-to-date clinical documentation while adhering to infection control and compliance standards. • Support the organization’s quality improvement and scheduling initiatives to ensure care is consistent, compassionate, and efficient. • Help patients and families navigate medication administration, hospice philosophy and services, symptom and pain management, and end-of-life processes and expectations. About You Qualifications - What You'll Bring: • Active RN license in your state (or eligible to obtain). • 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or similar fields. • A genuine hospice heart: compassionate, empathetic, and patient-centered. • Comfortable providing care in diverse settings, including private homes and facilities. • Strong communication and critical thinking skills in emotionally sensitive situations. • Valid driver’s license, auto insurance, and reliable transportation. • Current CPR certification required. Preferred Experience (Not Required): • Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac). • Admissions RN, wound care, float pool, travel nursing experience. • Familiarity with terminally ill patients or serious illness support.
