As a Hospice Visiting Nurse (RN), you will: • Provide essential support to patients and their families on their end-of-life journey. • Conduct comprehensive assessments to effectively manage symptoms and set goals for end-of-life care. • Work collaboratively as part of an interdisciplinary team that addresses the holistic needs of patients. • Develop and maintain strong therapeutic relationships through effective communication with patients and their loved ones. • Employ cutting-edge technology for precise documentation of skilled services rendered. Qualifications: • Current Registered Nurse with a valid CT RN license. • At least one year of RN experience; homecare or hospice experience is a plus. • Valid driver's license and reliable transportation are required. • Proficiency in computer usage is necessary.
