We are seeking a passionate Weekend Hospice Clinical Team Manager to join our mission of providing exceptional homecare services in Connecticut. Key Responsibilities: • Supervise and manage the hospice program to deliver high-quality, cost-effective care aligned with Hartford HealthCare at Home's strategic goals. • Provide guidance and support to staff in clinical and case management decisions focusing on excellence in end-of-life care and symptom management. • Facilitate the professional development of staff through home visits, evaluating clinical performance and implementing development plans as needed. • Ensure compliance with regulatory policies and procedures while promoting cost-effectiveness and optimal clinical outcomes. • Monitor team performance against clinical and operational benchmarks and implement corrective actions as necessary. • Utilize clinical software to track education opportunities, service utilization, and to oversee patient progression and team performance outcomes. • Resolve patient care issues by collaborating with internal and external stakeholders across regions. • Oversee daily patient assignments for the clinical staff based on client needs and staff competencies to maximize team productivity. • Coordinate referrals and ensure timely initiation of care as well as the delivery of adjunct services. • Evaluate and update individualized care plans to address continually assessed patient needs. • Ensure effective communication with all healthcare providers involved in the care plan of each patient. • Assess staffing requirements and coordinate hiring to respond to service demands in assigned areas. • Lead hiring, training, coaching, and performance evaluations for staff according to agency timelines. • Analyze team and patient metrics to align with agency initiatives. • Participate in educational programs regarding hospice and palliative care for students and peers. • Anticipate and respond to customer needs with professionalism and efficiency. Qualifications: • Active registered nurse license in the State of Connecticut. • Diploma in nursing is required; a Bachelor's degree in nursing is preferred. • Experience in hospice care, including familiarity with OASIS and case management certification, is preferred. • Basic keyboarding skills and proficiency in managing electronic calendars and emails.
