Role Description
This is a full-time hybrid role for a Hospice Nurse based in Glendale, AZ, with flexibility to conduct some tasks from home. The Hospice Nurse will be responsible for providing direct patient care to individuals in their homes, managing medical and palliative care needs, creating care plans, and offering support and education to patients and their families. The role involves close collaboration with the care team to ensure the highest standards of care and comfort. Regular documentation and communication regarding patient progress and needs will also be essential.
Qualifications
• Proficiency in Nursing, Medicine, and Palliative Care
• Experience with Home Care and Direct Patient Care
• Strong communication, empathy, and organizational skills
• Active Registered Nurse (RN) license in Arizona
• Ability to work both independently and collaboratively within a care team
• Familiarity with hospice and end-of-life care policies and guidelines
• CPR certification is preferred
• Valid driver’s license with reliable transportation
