The Nurse Practitioner (NP) is responsible for conducting Face-to-Face Encounters as defined by Medicare to verify patient eligibility for hospice services. This role ensures that all prospective patients receive timely access to hospice care when they meet Medicare's established criteria and Conditions of Participation.
Minimum Qualifications
• Current Registered Nurse (RN) and Nurse Practitioner (NP) licensure in good standing in the practicing state (Oklahoma, Texas, Kansas, or Colorado) and/or other company policy:
• * Oklahoma: Must hold an Advanced Practice Registered Nurse (APRN) license with prescriptive authority (if applicable), issued by the Oklahoma Board of Nursing.
• Texas: Must be licensed as an APRN by the Texas Board of Nursing and have prescriptive authority as delegated by a collaborating physician in accordance with Texas law.
• Kansas: Must have an active Kansas APRN license with appropriate national certification. Prescriptive authority requires application and approval by the Kansas State Board of Nursing.
• Colorado: Must be licensed as an Advanced Practice Nurse (APN) with full prescriptive authority if applicable, through the Colorado Board of Nursing.
• Graduate of a National League for Nursing (NLN) accredited nursing program.
• Completion of an NLN-accredited Nurse Practitioner course of study with successful board certification in the applicable specialty area.
• Preferred: 6 months to 1 year of management or supervisory experience, ideally in a hospice or healthcare setting.
• Compliance with professional standards and clinical best practices.
• Must be physically and mentally fit, with a pre-employment health clearance.
• Valid driver's license with auto insurance as per state/agency requirements and access to a reliable vehicle.
• Strong flexibility, cooperation, and adaptability in fulfilling duties.
• Satisfactory professional references.
• Excellent observation, communication, and interpersonal skills.
• Self-directed and able to work independently with minimal supervision.
Job Responsibilities and Duties
• Receive and process Face-to-Face encounter requests.
• Review clinical documentation and assess patient decline, recent changes, lab results, etc.
• Perform physical assessments during Face-to-Face encounters.
• Evaluate and verify that the patient's medical condition aligns with Medicare Local Coverage Determinations (LCDs) and hospice eligibility.
• Provide timely, accurate documentation of findings to the hospice team.
• Ensure that all clinical records comply with agency policies and federal, state, and local regulations.
• Uphold and reflect the agency's mission, vision, and standards of care in professional conduct.
• May be assigned additional job-related duties as necessary.
