Responsibilities/Duties (Including but not limited to):Direct Patient CareProvides hospice care visits and coordination of care in various settings (home, ALFs, SNFs).Assists with diagnosis, treatment, and management of acute and chronic health conditions.Interprets, evaluates, and communicates diagnostic findings.Prescribes medications, including controlled substances, in collaboration with Medical Directors and PCPs.Participates in rotating on-call responsibilities to ensure 24/7 medical coverage.Care Coordination & CollaborationCollaborates with referral sources and PCPs to ensure quality services.Consults with referring physicians, the hospice team, and other providers to contribute to the patient’s plan of care.Facilitates and documents clarification of patient and family goals of care.Maintains effective communication with coworkers, interdisciplinary team, patients, and families.Performs face-to-face visits to support hospice admissionsEducation & OutreachAssists in outreach activities to promote and educate about hospice services.Participates in community education through student/resident/fellow rotations, lectures, research, and presentations.Educates co-workers, facility staff, and the community on hospice care.Documentation & Quality StandardsPrepares and maintains accurate patient records, charts, and documents to support medical practice and reimbursement.Adheres to confidentiality practices regarding patients, families, staff, and the organization.Completes death certificates as required for Hospice of the Piedmont patients Qualifications:Licensed to practice in the Commonwealth of VirginiaCertified as a Clinical Nurse Specialist or Nurse PractitionerCertification by a national body (ANCC, NBCHPN, AANP)Preferred: experience in hospice/palliative medicine, clinical nursing care in geriatrics, oncology, or long-term care.
