Job Description

On-Call Registered Nurse (7 On / 7 Off) After-hours support. Mostly remote. Critical impact. The On-Call Registered Nurse provides after-hours triage, clinical guidance, and urgent in-person visits as needed for hospice patients receiving care in home or facility settings, supporting comfort, safety, and continuity of care. Schedule, Hours & Territory • Dedicated on-call role with a rotating 7 days on / 7 days off schedule • Mostly remote role with field support as needed • Coverage hours are 4:30 PM to 8:30 AM • Tri-Cities area (Bay City, Midland, & Saginaw) Benefits • Medical, dental, and vision coverage • 401(k) with employer match at 100 percent on the first 3% and 50 percent on the next 2% • Health Savings Account eligibility with employer contribution up to $500 for individual coverage and $1,000 for two-person or family coverage • Company-paid Life and AD&D • Voluntary disability, accident, critical illness, and hospital indemnity options • Employee Assistance Program, WillPrep, and travel assistance resources • Tuition reimbursement • Paid time off and paid holidays • Mileage reimbursement per company policy Key Responsibilities • Triage patient and caregiver calls after hours • Provide clinical guidance and symptom management • Complete urgent in-person visits as needed • Coordinate care with interdisciplinary team members • Document all assessments, interventions, and communication accurately and timely in the EMR HCHB (Homecare Homebase) Qualifications PROFESSIONAL EXPERIENCE/QUALIFICATIONS: • Help optimize the capital structure and support strategic initiatives. • Maintains the agency’s mission, philosophy and core values. • Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management. • Always maintains patient confidentiality, including all HIPAA regulations. Regulatory Requirements • Licensed or registered in the state in which he/she currently practices.. • Must pass a criminal background check & MVR check. • Completed health statement acknowledging ability to perform the duties of the position. • Valid state driver’s license. • Proof of current automobile insurance. • Current CPR card; On-line CPR certification accepted; Hands-on CPR certification preferred. • TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter). Industry Experience • Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural and religious modes. Completes Hospice training program. • Active patient contact within past three years, preferred. • Minimum 2 years of experience as a registered nurse, preferred Education • Graduated from an accredited school of nursing • Currently licensed as a registered nurse to practice in the state.
