What You'll Do as a Hospice RN / RN Case Manager: • Serve as the primary point of contact for patients and families, guiding them through their hospice journey. • Deliver and document skilled, hands-on nursing care according to each patient's personalized Plan of Care—in their home or other settings. • Conduct initial and ongoing assessments to monitor patient conditions and adapt care as needed. • Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, and chaplains—to develop personalized care plans. • Provide emotional, physical, and spiritual support to patients and their loved ones, offering education, comfort, and bereavement guidance. • Teach caregivers how to safely and confidently care for their loved ones, offering practical skills and emotional support. • Communicate essential updates on patient conditions to the care team, ensuring timely interventions. • Maintain accurate clinical documentation while following infection control and compliance standards. • Support our quality improvement and scheduling initiatives to ensure care is compassionate and efficient. • Help patients and families understand navigation of topics such as medication management, hospice philosophy, symptom relief, and end-of-life processes. About You: • Active RN license in the state of employment (or eligible to obtain). • 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or similar fields. • A compassionate, empathetic, and patient-centered approach to care. • Comfortable providing care in diverse settings, including private homes and facilities. • Strong communication and critical thinking skills, especially in sensitive situations. • Valid driver's license, auto insurance, and reliable transportation. • CPR certification required. Preferred Experience (Not Required): • Hospital RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac). • Experience as Admissions RN, in wound care, float pool, or travel nursing. • Experience with terminally ill patients or serious illness support.
