Hospice Registered Nurse

Join us in providing personalized, meaningful hospice and palliative care to those facing life-limiting illness. We ensure dignity, comfort, and compassion for both patients and their families.
Role: Hospice RN / RN Case Manager
• Serve as the main contact for patients and families, offering guidance throughout their hospice experience.
• Provide and document hands-on nursing care according to each patient's Plan of Care, whether in their home or a care facility.
• Conduct initial and continuous assessments to track patient conditions and adjust care accordingly.
• Work with an interdisciplinary team - including doctors, LPNs, CNAs, social workers, and chaplains - to develop individualized care plans.
• Offer emotional, physical, and spiritual support to both patients and their families, providing education and guidance through bereavement.
• Train caregivers to safely care for their loved ones with practical skills and emotional support.
• Update the care team on patient conditions to ensure timely interventions.
• Keep accurate clinical documentation and follow compliance and infection control standards.
• Support quality improvement and scheduling to ensure consistent and compassionate care.
• Educate families on important topics such as:
• Medication administration
• Hospice philosophy and services
• Symptom and pain management
• End-of-life processes and expectations
About You: Qualifications
• Active RN license or eligibility for licensing.
• Minimum of one year of RN experience in specialties like hospice, home health, ICU, oncology, or geriatrics.
• Empathy and compassion focused on patient-centered care.
• Ability to provide care in diverse settings.
• Strong communication and critical thinking skills, especially in sensitive situations.
• Valid driver's license, auto insurance, and reliable transportation.
• CPR certification is required.
Preferred Experience (Not Required):
• Experience in Med-Surg, ICU, ER, PACU, telemetry, or cardiac nursing.
• Admissions RN, wound care, float pool, or travel nursing experience.
• Experience with patients who have terminal illnesses or serious conditions.
