Position: Hospice Registered Nurse
Position Type: Full-Time
Remote/Virtual Position: No
Coverage Area: Hamden, New Haven and Orange
Salary: $90,000 - $105,000 / year
Schedule: Monday - Friday 8 am - 5 pm plus one weekend day back up on call every six weeks
$10,000 Sign on Bonus

Qualifications:
• Associates degree preferred
• Registered Nurse license required in state(s) of practice
• Current and valid CPR certification preferred
• Minimum one year of nursing experience preferred
• Experience in hospice, home health, or relevant nursing fields is advantageous
Key Responsibilities:
• Conducting comprehensive nursing assessments and developing personalized care plans with the interdisciplinary team (IDG) to address all patient needs.
• Documenting care and interventions accurately and promptly to ensure compliance with regulations.
• Coordinating care activities including scheduling visits, medication management, and facilitating communication between healthcare providers.
• Offering education and support to patients and caregivers and responding to urgent needs during on-call assignments.
• Supervising LPNs, LVNs, and hospice aides, participating in team meetings, and promoting ongoing clinical education.
• Evaluating patient status through phone, telehealth, or in-person visits to assess needs for additional services.
